I, the undersigned parent/guardian, hereby grant permission to the Iron City Forge Hockey Club, LLC to use my child’s name, image, and hockey-related information for promotional purposes in connection with the Iron City Forge Youth Player of the Week program.
This release authorizes the use of photographs, video, biographical details, and team-related information in, but not limited to, the following:
Social media posts (Facebook, Instagram, X, etc.)
Team website and newsletters
Promotional graphics, posters, or game-day announcements
I understand that:
1. My child’s information will only be used to recognize and highlight their hockey achievements.
2. No monetary compensation will be provided for participation in this program.
3. This consent may be revoked at any time by submitting written notice to the Iron City Forge Hockey Club, LLC at info@ironcityforge.com.
By signing below, I acknowledge that I am the parent or legal guardian of the minor named and that I have read and fully understand this release.
